This form is for students, staff and community members of the Sports Centre to provide their notification to cancel.
Annual, one- or three-month memberships cannot usually be cancelled except in circumstances listed in the Membership Agreement in the Terms and Conditions. They will automatically expire upon the end of the membership term.
